Consultation, advice and support on the health needs of vulnerable children and young people, to include vulnerable children and young people at risk of social/ educational exclusion including:

 

  • ‘Looked after’ children (children who are under the care of the local authority).
  • Children’s residential homes.
  • Children within the secure unit.
  • Individuals excluded from mainstream education e.g. attending Solutions 4 or alternative placement.
  • At risk of/ actual youth offenders.
  • Individuals in extremely complex circumstances.
  • Supervision for other health care professionals.
  • Training as part of a multi-agency strategy.
  • Liaison and networking to develop clear pathways for access to health services to improve health outcomes.
  • Direct work with children, young people and their families -This includes assessment, joint work with other professionals and short, focussed pieces of therapeutic intervention. Contact with individuals usually occurs if a child is looked after and our team had been requested to complete the Review Health Assessment, or they could be accessing a drop-in session which we provide to individuals who are in residential care. We also offer a similar role to that of a school nurse where education is provided under Solutions 4 provision and regular drop-ins are available for young people to access.
